Hallo mấy đứa,
Khổ thân chú A - lại bị panic tấn công mạnh sáng nay, muốn chết ngay trong ngày. Út cho chú A nói chuyện với 3 em của mình. Chú Rob nói út phải gọi urgent care. Bác sĩ urgent care đã tới, và quyết định gọi đội Crisis Team để họ tới xem cần cho chú A uống thêm thuốc gì cho êm trong ngày hôm nay.
Rất may là chú A đang nói chuyện điện thoại với cô Ivonne, út để speaker phone cho cô Ivonne khai bệnh với bác sĩ dùm chú A chứ chú A at this moment không biết nói gì luôn.
Lát nữa đội Crisis team tới, thường là psychiater đến, út sẽ lại gọi cô Ivonne để cô ta nói chuyện với dân professional care bằng tiếng Hòa Lan cho rõ ràng (giống lúc nãy, cô Ivonne was very helpful - cho bác sĩ biết toàn bộ tình hình - kể cả những info mà chú A hay né không nói khi ... không muốn bị chết hic.)

Hallo chị Hòa,
Suốt ngày 16, nấc cục phải đến 5 đợt, mỗi đợi dài ít nhất 2 tiếng. Ivonne đến thăm, nằm ngủ trên ghế salon là chính. Cuối buổi lẫn lộn, hỏi Ivonne ... có bạn trai hả. Rồi tưởng phải ngủ dưới nhà, nhưng cuối cùng Alouis đi lên thang gác được.
2:30Am Alouis thức dậy đi tiểu thì hết nấc, nhưng quay lại giường bắt đầu im lặng làm những hành động kỳ kỳ như ngồi trên giường từ từ cởi hết áo ra, đưa áo mới thì lẳng lặng mặc vào. Cởi áo xong thì từ từ cời quần ra, rồi lấy áo gối thò chân vào mặc, nghĩ rằng áo gối là quần. Rồi lại bị nấc lại.
Em phải gọi em trai Jos nhờ Jos thuyết phục Alouis nằm xuống giường. Cũng may sao tự nhiên em nhớ lời bác sĩ tâm thần của đội Crisis Team nói là có thể cho uống thêm thuốc Xanax để calm A xuống. Em hỏi ý kiến Jos, Jos đồng ý phải cho A uống thêm thuốc đó.
A êm ngay, rồi ngủ, và hết nấc được vài tiếng. Giờ là 7AM, cơn nấc tấn công lại, quá sầu luôn.

Hallo mấy đứa nhỏ,
út báo cho mấy đứa biết là vì sáng nay cô làm cho home care đến thăm chú A, nói tình hình của chú A xấu nhanh quá.
Ngay sau khi cô ta ra khỏi nhà út, cô gọi cho bác sĩ gđ của chú A. Sau đó gọi ngay cho út : cả hai đều dời ngày gặp lại chú A sớm hơn (thay vì bs thứ 5 sẽ đến và cô ta thứ 6 sẽ đến thì cả hai sẽ đến thứ 4 là ngày mai)
Đến sớm hơn dự định như vậy có nghĩa là ngày chú A ra đi rất gần rồi đó mấy đứa.

Kể V nghe chuyện 3AM sáng nay. A thức dậy. H thấy A kỳ kỳ. H đọc với giọng hát "Yết đế Yết Đế Ba la Yết Đế Bồ đề Tát Bà Ha" theo lời khuyên của Thầy Tuệ Sỹ trong một pháp thoại để quét dọn không gian quanh mình.
A nghe, hỏi: "H hát gì vậy". H nói "Đây là hát ru đó". A nói: "Tui hổng buồn ngủ. Khỏi hát ru." H mắc cười quá, nhưng vẫn tiếp tục "hát". A khôn lắm, nói: "Không phải hát đâu, câu H hát có một ý nghĩa gì đó."

- Milou, the homecare's staff, will be back on Friday 21.01 at 1PM to help me to bath Alouis.
- Dr. Robertson, Alouis's family doctor, will pay Alouis a visit again on Friday 21.01 at 9:30AM
- Dr. Robertson will arrange a blood test at home for Alouis to see why he suffers the hiccup.
- Dr. Robertson decided to reduce morphine Fantanyl from 72.5mg to 50mg/every 3 days
- Dr. Robertson said she and others could take care of Alouis at home, but she also offered the service at hospice for Alouis whenever Alouis agrees to move there to stay.

Alouis wake up at 3AM, firm steps to the bathroom to wash his hands (quite weird action). When he saw me, he asked more than once why I gave me no medicine recently? I gave him an extra Xanax. He seemed pleased and went back to work with still heavy breathing.
I wake up at 6:45AM, feeling happy because hearing no hik, no heavy breath.
But when I look at his face, I saw his eyes stay open with no feeling again.
I asked him if he was tired, he shook his head meaning yes, and smoothed my hairs with his hand.
Now he's back to sleep.

Uit het bloedonderzoek blijkt dat Alouis bloedarmoede heeft als gevolg van de uitbreidende ziekte, Dat verklaart zijn vermoeidheid, zijn bleke kleur. Uit het bloedonderzoek geen verklaring voor zijn verwardheid, de urine is ook in orde. Alouis vergeet steeds meer, Praten wordt moeilijker door verlies van energie. Door de progressie van de ziekte kunnen er ook op andere plaatsen uitzaaingen zijn maar dat weten we niet. Hij vroeg hoe het nu verder gaat,; hij zal steeds vermoeider worden, bij klachten van pijn, kortademigheid, onrust passen we de medicatie aan. Hij heeft geen trek meer om te eten maar eet toch omdat het in het systeem zit. Minder eten / drinken zal de achteruitgang versnellen.

Dag Rob,
Dr. Robertson just called. She will replace Abstral (for de hik) with another medicine which will be collected by Janine and Marcel at the hospital's apotheek (due to the weekend, the apotheek de Fronten is closed).
Dr. Roberstson also said the "stomach pain"/ "belly pain" that Alouis complained last midnight was probably caused by the tumor she felt in his belly while examining him yesterday. That pain and de hik made him stayed awake almost the whole night last night. Luckily, no panic happened.
We are waiting the delivery of 2 beds which can be any time from now till 2PM.

The huisarts is net left. Some changes in treatment:
- Fantanyl: increase back to 72.5mg/3 days
- Extra pill to sleep at night
- Probably night shift staff from Thuiszorg will come to sleep during the night at hotel Soendastraat 1
- Probably Naproxen, the pain killer which is not friendly with the stomach, will be used again.

Sáng nay bác sĩ cho tăng liều morphine lại vì Alouis than đau quá. Thế là A lại bị ngủ nhiều, đầu óc lơ mơ, lần đầu tiên xè dầm trong quần một chút.
Cô nurse của Home Care mới text cho út:
"The doctor called me to say that the situation is not getting better… I will ask the office to search for a nurse that will stay at you by night from 23 till 6 is that oké for you?"
út đã hỏi gia đình chú A là để người lạ ngủ trong nhà mình có an toàn không. Mọi người nói không sao hết.
Tình hình đầu ngày lúc 1AM là thế.

Hiệp lực cầu nguyện và sức mạnh tình thương của các anh chị và xấp cháu dành cho hai út hiệu quả vô ngần. Trước mắt là có hai thiên thần xuất hiện: Bà bác sĩ Robertson tự nguyện đến tận nhà khám bệnh cho Alouis thứ 7 và chủ nhật vừa rồi. Cô Milou hôm nay là ngày nghỉ mà vẫn theo dõi tình hình và quyết định đến thăm A. Nhờ vậy mà út vẫn có đầy năng lượng để vẫn nấu ăn cho các em Alouis khi các em xuống thăm :)
Hai út cảm ơn "ai hết" nhiều lắm lắm.

[11:49 PM, 1/24/2022] Huong: Alouis wake up to pee again and acted strange : refused to lie back down to bed, saying he wants to go ... I asked go where, he gave no answer. Step by step, I guided him first sat, then lied down. I pulled up the bedhek. How he's trying to get out of bed again, telling he trys to shovel a certain kind of things with other people. The delier seems return. If he is still behaving like that, can I give him Xanax?
[12:02 AM, 1/25/2022] Huong: His urine is still there ... dare not to leave him because he keeps on trying to get out of bed.
[12:12 AM, 1/25/2022] Huong: His body is lightly shaking but he already fell asleep. Time to throw away his urine and get to sleep myself. Tomorrow I will tell Dr. Robertson about the shaking body.
[3:27 AM, 1/25/2022] Huong: A peed again some in pants, some in the emmer of the toillet chair. I had to use the diaper this time instead of the sanitary pad and changed new underwear for him. It's good that he fell back to sleep again right away.
[8:02 AM, 1/25/2022] Huong: A just tried again get out of bed to pee. The diaper works - he is not wet. He peed in the toillet chair and refused to get back to bed but stood still for a while ant intended to step to the door. It seems the delier has not stop yet. Now he's put in bed, lying down with de hik. I go to take a shower now. A new day begins. Have a nice day at work, Ivonne.

Good morning Dr. Robertson,
A long, severe hiccup started around 7:40AM this morning.
Alouis complaint that he was sweating a lot (not true), asking to drink a lot of water (very unusual)
Last night at around over 12PM, he urinated fully to his diaper, leaking to his pyjamas pants.
He cooperated well with the nightshift staff named Jasna. I myself had enough sleep. I don't have body ache now because I did not have to help A to sit up, to step to the toilet chair and so on.
A just asked me: "Huong, what you want me to do?" and told me "We must go home".
He also continuously groaned that he got belly ache. Then he saw me eating a banana, he asked me to share. After eating almost "my" banana, it seemed that he was back to normal, sitting a bit to watch TV and went back to bed to sleep.
Talk to you more on phone this morning.

Hallo Jos,
1. Dr. Roberstson just called. Her husband and one of her 3 children got positive tested. She herself got negative tested but she prefered not come to our house tomorrow to pay Alouis a visit but only call and let another colleague to come to our house.
2. Milou came to our house at 1:00PM to talk to Alouis. She said there will be a different night shift staff to come tonight. After that we can decide to use this service every night or every 2 nights. To tell you the truth, this service helps me a lot. Milou will see you tomorrow, too after helping Alouis to have a shower.

For your reference, Alouis has not urinated since 10PM last night.
At around 5:55AM this morning, the nightshift nurse tried to let him drink water but he could no longer swallow the water down.
He is now sleeping with no hiccup (which returned last night and last one time 3 hours)

Alouis vẫn eager to sống mặc dù chỉ uống nước và ăn 1/2 trái chuối. Bác sĩ hôm nay cuối tuần mà cũng ráng đến để nói chuyện với Ivonne. Bây giờ út vẫn tiếp tục cho A uống những thuốc chính, khi nào không uống được nữa thì sẽ cho thuốc vào qua đường kim.
Today was with Alouis and Huong. Huong had told the doctor that I was coming. The GP asked if she could come by and talk to Alouis again. During the conversation she asked again if he wanted to go to sleep. He said several times that he did not want that. I asked him if he remembered our mother. And if he wanted to. He said he didn't remember.
We told him we'll do it the way he wants it. However, the medication, paracetamol, if he finds it annoying, can be dropped. If he indicates pain, he may take extra medication. Huong does not need to be careful about this. Extra Xanax is also allowed if he is restless or in a panic.
The doctor told Huong to give him water, which should be small amounts. Not a glass half full, but a bottom. Alouis can pee himself again. Catheter is not going to do that. It's also not going to give him more comfort. Chances are he'll pull out. And that is very painful. He said himself, I was in a lot of pain yesterday and I don't want that anymore.
He makes it very clear that he doesn't want to eat anymore. And the doctor also says, that is no longer necessary. When the doctor left, he fell asleep. I stayed with him. Huong has been upstairs and has been in contact with family. Huong's father doesn't recognize everyone anymore either. But Huong did, her father sent best regards to Alouis.
At one point he woke up. Was a bit confused. Wondered where he was. I said he was home. And I visit him. Mieke is coming tomorrow, Rob came yesterday. And Jos Thursday. He wanted everyone to come.
Reported being tired. And that he no longer knew. I only said when you're tired, go to sleep. Then he said yes I'm going to sleep.
GP did say that if he can't swallow anymore, there will be another conversation. I said that's good.

A surprise visit: Dr. Robertson and Milou decided to come to our house at 5:30PM today since they were informed by me that Alouis kept on sleeping a lot since last night till late this afternoon without taking any medication. They planned to put needle to give him medicine via vein. But before they came, A was alert enough to take Xanax as Milou's advice and when they were here, A was alert enough to talk to Dr and to take Olanzapine. Now they decided to go home without doing anything for him. However, tomorrow, if he's much less alert than today, they will do what they planned to do late this afternoon.

He told me that he doesn't understand why he has to drink with a straw. Then he told me that he does not want to drink olanzapine. I said you don't drink olanzapine. Then he talked about a yellow pilletje. I said to him the huisarts says that he has to take medication for pain and unrest. Than he said i took the medication. He says it is raar, means strange.

Chú Alouis báo động giả hoài, mắc cười ghê. Chuyện là hôm qua Milou tận mắt thấy chú A nuốt khó rồi, báo bác sĩ. Bác sĩ đến, nói chuyện với chú A, xin phép được gắn sẵn kim vào đùi, nếu không cho chú A nuốt viên thuốc được tối nay thì sẽ bơm thuốc an thần và thuốc ngủ cho chú A được ngủ ngon.
Chú A lơ mơ đồng ý. Nhưng ... chỉ 1 tiếng sau, chú A nói út đưa chai nước cho tui uống, tui không uống bằng ống hút và ... uống cái ót, uống xong còn cười cười nhìn út nói "See!"
Rồi sáng sớm nay đã đủ khỏe để đưa tay xuống đùi, cố tháo băng keo mà bên dưới là kim ghim sẵn.
Quá quậy luôn mấy đứa ơi.

Na,
Bác sĩ vừa chích mũi thuốc ngủ đầu tiên cho chú Alouis sau khi chú Alouis đồng ý cho phép cho thuốc vào.
Loại thuốc này sẽ được cho vào vein mỗi 4 tiếng. Chú A sẽ ngủ và ra đi.
Út đã dặn chú Alouis trước khi thuốc được cho vào người là thấy luồng ánh sáng thì theo đó mà nhập vào vũ trụ, không được luyến tiếc gì nghe chưa. Chú A ừ.
Mũi thuốc đầu tiên đã được tiêm cách đây 5 phút mà tay chú A còn đi tìm cái remote control cái giường để bấm. Bó tay chú A luôn.
Gia đình cũng đã được nghe cuộc đối thoại giữa bác sĩ và chú Alouis. Gia đình có hỏi là út có cần ai xuống ngủ không.
Út nói không vì tối nay có cô nurse trực rồi.
Ngày mai người nhà chú Alouis xuống, bác Trúc xuống.
Vậy ha, coi như là từ từ mình tạm biệt chú A đó.

Chú Alouis qua đời lúc 2:15AM ngày 05 tháng 02 năm 2022. Chú Alouis đi thanh thản. Hai người em út của chú A ngủ vớ út tối hôm qua. Ba chị em bình tâm lo cho chú A. Bác sĩ đến ... lé mắt với không khi đầm ấm mà bà ta cảm được khi bước vào nhà để ký giấy "báo tử" (luật bên này bác sĩ đến check và ký xác nhận chú A thật sự đã qua đời)
